Income just $11,000 lower would have put you in the tier below. Even $1 over a threshold means the whole surcharge for that tier.

What this means

Medicare uses your income from two years before: your 2024 tax return sets your 2026 premiums. A one-off jump in income, from selling a home or investments or a large Roth conversion, can raise your premiums two years later.

If your income has fallen since because of a life-changing event, such as retiring, losing a pension, marriage, divorce or the death of a spouse, you can ask Social Security to use your newer income with Form SSA-44.

Assumptions in this estimate
  • For 2026, from CMS. Part B’s annual deductible is $283. The 2027 amounts, based on 2025 income, are announced in November.
  • Income is modified adjusted gross income: your AGI plus tax-exempt interest. Roth IRA withdrawals and qualified charitable distributions don’t count.
  • The Part D surcharge is added to whatever your drug plan or Medicare Advantage plan charges, and is paid to Medicare.
  • If you have Medicare Advantage, you still pay the Part B premium, IRMAA included.
