Medicare IRMAA Calculator

See your 2026 Medicare Part B premium and Part D surcharge for your income, which IRMAA tier you’re in, and how much lower income would drop you a tier. Built on the CMS 2026 figures.

$

Adjusted gross income plus tax-exempt interest

Your Part B premium in 2026

$284.10 a month

Including $81.20 of IRMAA, plus $14.50 a month on top of your Part D plan’s premium.

Part D surcharge
$14.50/mo
Extra a year
$1,148
Next tier above
$137,000

Income just $11,000 lower would have put you in the tier below. Even $1 over a threshold means the whole surcharge for that tier.

What this means

Medicare uses your income from two years before: your 2024 tax return sets your 2026 premiums. A one-off jump in income, from selling a home or investments or a large Roth conversion, can raise your premiums two years later.

If your income has fallen since because of a life-changing event, such as retiring, losing a pension, marriage, divorce or the death of a spouse, you can ask Social Security to use your newer income with Form SSA-44.

Your Part D IRMAA$14.50

Assumptions in this estimate
  • For 2026, from CMS. Part B’s annual deductible is $283. The 2027 amounts, based on 2025 income, are announced in November.
  • Income is modified adjusted gross income: your AGI plus tax-exempt interest. Roth IRA withdrawals and qualified charitable distributions don’t count.
  • The Part D surcharge is added to whatever your drug plan or Medicare Advantage plan charges, and is paid to Medicare.
  • If you have Medicare Advantage, you still pay the Part B premium, IRMAA included.

How the calculator works

  1. Your income: modified adjusted gross income from your 2024 tax return: your AGI plus any tax-exempt interest.
  2. Your tier: the IRMAA tier for your filing status.
  3. Your premiums: the Part B premium for that tier, the Part D surcharge added to your drug plan’s premium, and the extra cost a year.

IRMAA tiers for 2026

Single (2024 income) Joint (2024 income) Part B a month Part D extra Extra a year, each
Up to $109,000 Up to $218,000 $202.90 $0.00 $0
Over $109,000 to $137,000 Over $218,000 to $274,000 $284.10 $14.50 $1,148
Over $137,000 to $171,000 Over $274,000 to $342,000 $405.80 $37.50 $2,885
Over $171,000 to $205,000 Over $342,000 to $410,000 $527.50 $60.40 $4,620
Over $205,000, under $500,000 Over $410,000, under $750,000 $649.20 $83.30 $6,355
$500,000 or more $750,000 or more $689.90 $91.00 $6,936

Married people filing separately who lived together pay $649.20 over $109,000 and $689.90 from $391,000.

Each tier has a hard edge: $1 of income over a threshold means the whole surcharge for that tier, for each person on Medicare.

Examples

A single retiree with $120,000 of income in 2024 pays $284.10 a month for Part B in 2026, and $14.50 a month on top of their drug plan: $1,148 a year more than the standard premium. With $11,000 less income they’d have paid the standard premium.

A married couple, both on Medicare, with $300,000 of joint income pay $405.80 each for Part B and $37.50 each extra for Part D: $5,770 a year more between them.

A one-off gain costs you two years later

A single filer whose income is usually $90,000, with a one-off gain in 2024, pays this much more for Medicare in 2026:

One-off gain 2024 income Part B a month Extra in 2026
$25,000 $115,000 $284.10 $1,148
$50,000 $140,000 $405.80 $2,885
$100,000 $190,000 $527.50 $4,620

A home sale with a gain above the exclusion, a big Roth conversion, a large IRA withdrawal or a year of capital gains can all push income up for one year. Spreading them over several years, below a threshold, can avoid the surcharge. The Roth conversion calculator shows the tax on a conversion.

Frequently asked questions

What is IRMAA?

The income-related monthly adjustment amount: an extra charge on Medicare Part B and Part D premiums for people with higher incomes. In 2026, it applies to single filers with 2024 income over $109,000, and joint filers over $218,000.

What is the Medicare Part B premium in 2026?

The standard premium is $202.90 a month, and the deductible is $283 a year. With IRMAA, the premium ranges up to $689.90 a month.

Which year’s income is used?

Your tax return from two years before: 2024 income sets 2026 premiums. That’s why a big one-off gain, such as selling a home or a large Roth conversion, can raise your premiums two years later.

Can I appeal IRMAA?

Yes, if your income has fallen because of a life-changing event: stopping or cutting back work, losing a pension, marriage, divorce, or the death of a spouse. File Form SSA-44 with Social Security, with proof of your lower income.

How can I keep my income under an IRMAA threshold?

Plan withdrawals and Roth conversions around the thresholds, give to charity straight from an IRA with qualified charitable distributions (which don’t count as income), and spread large capital gains over several years. Even $1 over a threshold means the full surcharge for that tier.
